How Are We Doing?

At Stanford Childrens Health, we want your experience to be extraordinary. We value your feedback because it helps us improve and allows us to recognize the staff members who helped provide the best possible experience and outcomes for you and your family.

Comments, questions or concerns

If at any time you have comments, questions or concerns about your child’s care, please talk to your nurse, physician or the manager of your hospital unit or complete our online form. You may also reach out to a social worker or chaplain, or contact Office of Patient Experience at (650) 498-4847.

For any concerns about patient safety or quality of care you don’t feel we were able to address, you may report your concern on the Joint Commission reporting page or by calling (800) 994-6610.

Recognize a staff member

Our doctors, nurses and administrative staff members are all committed to providing exceptional care. If you feel like someone on your care team went above and beyond, please help us recognize their dedication. You can use this form to share your experience.

The DAISY Award for extraordinary nurses recognizes the super-human work nurses do every day. You can nominate an extraordinary nurse with the following DAISY Award nomination form in English or use the DAISY Award nomination in Spanish. Nominations received by the 15th of the month will be considered for the following month's DAISY Award.
